Talryn:
    - Remove all guild contribution code.  It is removed in 6.0.
    - Update TOC for 6.0.
    - Fix bug with Add Menu Item code.
    - Fix issue with trying to rehook the menu.  Just require a restart.

    - Added new format and options to pick which formats are used.
