Paladin fixes:
- Create spell-specific tables of librams.
- Add WotLK librarms.
- Glyphed Seal of Light increases effective healing by 5%.
- Simplify the bookkeeping of factors that buff effective healing by wrapping it all up into a single factor "effectiveHealBuff".
- The librams all increase the spellpower of their respective spells, so make the calculation of the healing bonus more transparent and fix the Holy Light bonus calculation.

Fix Chain Heal size estimate:
* Improved Chain Heal and Purification talents are additive in their effects, not multiplicative.
* Unlike the totems for LHW and HW, the totems that affect Chain Heal increase the base amount healed, not the spellpower of the healing spell.  Move the totem bonus so that this is accurately computed in the effective heal.

Tidal Waves only affects the bonus healing, not the total effective healing, so move the multiplier into the bonus healing calculation instead.

Fix Tidal Waves calculation for shamans -- the bonus to Healing Wave and Lesser Healing Wave is a passive bonus and doesn't depend on whether the buff is up on the target of the heal.
