Updated the UIs to clear out the data fields when the forms are closed.
Added a database converter to help patch the databases when new version are rolled out.
Made the code case-insensitive when looking up records in the database.  Hopefully, this will prevent some issues with multiple records being created for the same individual.
Added a slash command help to the add-on start up.
