When entering stealth or targeting something while attacking it is not possible (e.g. picking up Myrrit during a dig), the target frame becomes faded as it were out of range and it stays that way forever, even when exiting stealth and attacking it.
Very easily reproducible on a druid:
1. Target a hostile mob, go into range and note the frame shows as "in range" (not faded).
2. Enter stealth -> the target frame becomes faded - at this point this behavior is already wrong. It should not fade just because of entering stealth.
3. Attack the mob from stealth -> target frame will stay faded forever, no matter the range.
Using v4.4.11, only noticed this today so the problem might have something to do with the client update that was pushed last night.
Turns out it's not stealth but cat form that triggers this issue.
I have found that when targeting an enemy, the target frame will sometimes fade out when I cast a spell, and then fade back in when I cast the next spell. Doesn't seem to be a particular spell.
I think it has to do with spells that are modified by talents.
As if the modified version of the spell would give an incorrect or invalid range - in my case, it's Moonfire - when it is talented (Lunar Inspiration), the target frame is correctly faded/unfaded in caster form, but becomes permanently faded in cat form. I temporarily worked arount this by manually setting Wrath as range check (default is Moonfire), but this is a faulty behavior nonetheless.
