Tune SkadaDeaths for better memory/cpu performance, and other general improvements
Change the logging to recycle entries, saving a large amount of unnecessary memory churn in combat
Log srcname explicitly and delay string processing to display time
Log only spellid when spellname is redundant, and fetch it as needed at display time
Add logging for ENVIRONMENTAL_DAMAGE (falling, drowning, fatigue, etc)
Add logging for SPELL_INSTAKILL (some special boss abilities)
