What steps will reproduce the problem?
1. Join a raid (10 or 25 man) and continue on in it for a while. Or log off then back on while in the raid.
What is the expected output? What do you see instead?
Expected: for the mod to pay attention to & understand the number you have set in the "Wait for certain amount of the raid to arrive before group and class buffing commences. Zero to always buff" option.
Instead: it doesn't recognize the number of raid members within range properly. Thus if you have that option set to any value other than 0%, it will never have its icon available to be clicked.
Typical tooltip text mousing over the buff icon when this happens with a full 10 or 25 man raid _all within range_:
ZOMGBuffs Waiting for 75% of the raid to arrive before buffing commences (0% currently present)
Any percentage you set the "Wait for certain amount of the raid..." option to greater than 0% will cause this problem when everyone is within range. Set it to 0% and you can once again click the ZOMGBuffs icon to buff.
This problem happens 100% (heh) of the time when you relog/reload the UI while in a raid. Also happens (but don't have the frequency data) when you don't reload/relog.
What version of the product are you using?
r176. Also happens with all versions back to v4.0 release 2 (when I decided to try this again after the patch hit).
Do you have an error log of what happened?
No errors.

This issue never occurred in versions used in WoW 3.x & earlier (r156 & prior). I started again with ZOMGBuffs with v4.0 release 2 with no saved variables (deleted the 3.x-era one before starting) in WoW 4.0.1 live.
