6.0 and Warlords of Draenor
The addon is updated for 6.0 but the Disease Strength feature has been removed. In 6.0+ Death Knight diseases will no longer snapshot their strength. They update dynamically so there is no need to track their strength.
The Soul Reaper vs. Howling Blast functionality is also not needed now but it is still in the addon. You can disable it as Soul Reaper will always do more damage than Howling Blast. It will just show the Soul Reaper icon when appropriate.
Overview
DK Diseases will track your diseases on your target or focus.
You can choose either the block or bar display. It is highly configurable so check the settings to adjust the display to your preference.

Any chance you can add cooldown spiral to the block frame?

I seem to have found a bug of sorts. I've been exlcusively trying to fix this for the past hour, but have had zero luck. It appears that the Soul Reaper options only work for Frost. I've uninstalled/reinstalled the latest release today, I've changed profiles, reset profiles, changed class specializations, changed class talents and glyphs, change class specializations slots, disabled ALL other addons, reloaded, closed the game, etc. No matter what I do, I can not get Soul Reaper to appear for Unholy or Blood. It works perfectly fine for Frost.
Another thing that I noticed for Soul Reaper. By default, it is set to 35%. However, Unholy's threshold is 45%. The percentage does not change if I switch to Unholy. I can manually set the slider to 45%, but it stays at 45% when I switch back to Frost. I'm not sure if this is a side effect of the Soul Reaper options not working fully for Unholy or not, but it was something else I noticed. Any way to get these thresholds to automatically set themselves for each specialization?

Yes, it only works for Frost. It was meant to indicate whether you should use SR or HB. In MoP at the end of the expansion, if you had enough mastery then HB was more damage than SR. The addon would dynamically calculate it since sometimes procs would push you over that tipping point.
I should probably explain the background of the addon as it will probably explain what seems like odd behavior. It was originally written to handle advanced play such as disease strength management, including the Festerblight play style. When 6.0 was released the primary reason for this addon was gone. It turned out that a lot of people were still trying to use it as a basic disease tracker. To help them, I updated the addon.
I can look at fixing some of these issues but most of my addon development time is spent on Compact Runes. It provides a lot of this tracking and is a full-featured rune addon. It's worth looking into it. I do realize some people are already tied to another rune addon and it is rough to make the change as you have to break an old habit and re-learn a new one.
I actually had compact runes for quite awhile, but I could never get used to the rune layout. I had to switch back to something else. I'm currently using Engraved right now for my runes. In my personal opinion, it has been the best by far for rune tracking (nothing against compact runes). This mainly has to do with the customizability of the runes, the ability to handle each rune individually, and overall visibility. It works perfect for my UI.
I was originally using MagicRunes for just disease tracking because it's disease bars were very nice. Downside to it was there is no Necrotic Plague tracker. When you updated this addon, I immediately switch to it. As far as disease tracking go, it's simple, perfect, and it has bars! The Soul Reaper option was a bonus really. It would be really nice if it worked for Unholy and Blood though.
Big thing is, not everyone is looking for an all-in-one. I would ask you not to abandon this addon. There are folks out there that are only looking for a disease tracker! This addon is really great for that.
Using the alpha, and so far the Necrotic Plague tracker looks good.
Two things I would like to see with this addon however:
1. Split the diseases! If one disease is not on my target, I do not want that block or bar to appear. Currently, it shows both, regardless if there is only one disease on the target.
2. With the bar option, please give scaling options with the icons. I can scale the bars just fine, but the icons stay tiny.
I can check into adding an option to separate the display of the diseases. They were dependent as a way to remind you to put up both diseases.
The issue with the icon size is a bug where it doesn't update its size until you reload the UI or log out and in. The latest alpha fixes this issue.
I appreciate it! This isn't an issue with Necrotic Plague, but I don't always use Necrotic Plague. When I don't, the disease bars being "separate" would be a huge benefit. I track each disease by the bars, and if one of the two bars is not there, I know I need to re-apply that disease. With how it is currently, I sometimes don't get a good enough look at the bar when I'm in the middle of the fray, and I catch myself not re-applying a disease when I should. It's probably more of an issue with me, but it would be very helpful.
Definitely would like to have same functionality for Necrotic Plague as you have for Frost Fever and Blood Plague.

That's sad.
The base tick computes what each tick of the disease would be but only includes your AP and damage modifiers. The effective tick includes Cinderglacier and crit chance. Since effective is increased by 1 + crit percent, you'll never see an actual disease value match the effective tick but it is how the addon accounts for an increase in crit which would help to boost your disease damage.
Those options are mostly informational. The color coding should help you to decide what to do. I normally just use the percent one to see the estimated increase in disease damage.

Just discovered this addon, and I like the idea. However, I'm confused about what I'm looking at. From what I've read, diseases shouldn't be refreshed until just before the last tick of the diseases; to do so otherwise is a dps loss. So why are the backgrounds yellow when the timers say, for example, 25 seconds? Yellow means refresh, correct? Five seconds after I applied the disease? What am I not understanding about the addon? I was hoping this would be a visual reminder of when to reapply because it's easy to get distracted in a raid fight, but I need to understand more of what I'm looking at before I can use it.
I will add a section to the description to better explain it. Most people seem to know what this does when they are looking for it, but it is possible some people haven't learned about this aspect of disease mechanics.
While the addon does track disease presence and time, the more advanced usage is to indicate disease strength. For Death Knights, diseases are "snapshoted". The value of their damage is calculated at that moment and it does not change until re-applied. So all ticks of the diseases do the same damage that was calculated at the moment it was applied. That does not change until they are re-applied.
This addon tracks the various inputs to the damage (attack power, mastery, etc.) and compares the current disease strength to what you would get if you reapply them. Red would mean there is no value in reapplying diseases early. Yellow means there is an increase. Green means it is an increase and is above a threshold. That threshold by default is around 9-10%. You care about that threshold because using a GCD / rune for a 1% increase in disease damage wouldn't be worth it compared to the damage of using it on another attack.
If you're using the addon, you would always apply diseases if they were not present or if the timer shows they may fall off very soon, but you may want to re-apply them before they expire too. This addon helps to track if you should re-apply them early based on the damage gain.
Thanks for the explanation; the idea of "disease strength" is something I wasn't aware of. I just always made sure to reapply the diseases either right before they fell off or very soon afterwards.
I would really like to use this addon for the Soul Reaper vs. Howling Blast indicator, but it does not seem to be working. I've tried various settings, but it just doesn't work. Even when I set it to Always Show and Value, the value stays static (it reads "60k - 499") no matter what trinkets, unholy strength from Rune of the Fallen Crusader, Pillar of Frost, etc. proc or change while I'm in combat. If you can get this to work, it could be extremely useful for dw frost dks seeking to maximize damage.
