I am running ver 2.2.1 right now of raid tracker, lua has been wiped before I installed your new version.
This is what im going to do for now to fix old raids DB update wise: if you have two events (which thankfully i recorded the instance info per event as well as per session) it looks to see if the second kill doesnt match the first iid, if so it sets the session inst details, and the first event inst details, to the second event inst details. This is flawed of course, ther are lots of ways the lookups could be wrong, but in many casses will set the difficulty correctly to start with.
This is beign fixed, actually already is for 2.2.2 to work correctly for new raids, i will also make sure the new session on new instance is aware that idiff matters and not jsut zone name. I cant key on instance id because that wont show til a boss kill, unless i can get the fake oen they use for display as you enter, so must be a dual key, zone and diff. I'm goign also to take the effective idiff reported back form the server, not the user idiff setting when you choose heroic for example, so you have to walk in for me to use that. I'll get somethign out for tonight and clean up with 2.2.3 which because il reconcile all this might mean new featuires implicity that are easy to add after i do this.
Indeed I can also confrim Noth the Plaguebringer is not recorded as a boss kill event, and the 4 Horsemen are recorded individually.
I think the 4 horsemen is more difficult an issue as you do actually kill 4 "mobs" then loot a chest which would be hard to code for I guess (order they are killed in varies from encounter to encounter, etc...) but if you can do some sort of state machine where if one of the "named" dies, it waits for the rest of the names in the list to die, then labels it as 4 Horsemen, that could work, dont know havent written Mods so dont know if its possible, I only know C++ :)

Our loot system gives points on attempts and kills, but I can't figure how to get raid tracker to record attempts as well as kills. What point on the slider will record attempts, and what might an attempt look like on the xml?
